A practical approach identifies processes that depend on a single person with no backup. That concentration of knowledge represents an operational risk regardless of organisation size.

Process documentation reduces dependence on an individual, but doesn't replace the need for genuine knowledge transfer through practice.

The greatest risk is the sudden loss of a critical-knowledge holder without a prepared replacement. That risk is reduced by a systematic knowledge-transfer plan.
